ACKNOWLEDGMENT OF RISK
I hereby acknowledge and understand that participation in the Oh Snap! Spring Slam basketball tournament involves inherent risks and dangers associated with a physical sport. These risks include, but are not limited to, injuries from falls, contact with other participants, and the potential for serious bodily injury or even death.

WAIVER OF LIABILITY
In consideration of being allowed to participate in the Oh Snap! Spring Slam, I, and if a minor, my parent/guardian, hereby agree to release, waive, discharge, and covenant not to sue Soul of the South Sports, its officers, employees, volunteers, agents, and other participants (collectively "Releases") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death, that may be sustained by me, or to any property belonging to me, while participating in such activity, or while in, on or upon the premises where the event is being conducted.

MEDICAL TREATMENT CONSENT
In the event of an injury or medical emergency, I give permission for tournament staff, volunteers, or medical personnel to provide emergency medical care. I agree to be financially responsible for the costs of such medical treatment.

PHOTO AND VIDEO CONSENT
I understand that during the tournament, photographs and videos may be taken of participants. I hereby grant Soul of the South Sports the right to use and publish these photos and videos for promotional purposes, including but not limited to social media, websites, and print media.

PARENT/GUARDIAN CONSENT (if participant is under 18)
As the parent/guardian of [Participant Name], I confirm that I have read and understood this form and consent to my child's participation in the Oh Snap! Spring Slam, and agree to all the terms on their behalf.

AGREEMENT
By signing this waiver, I acknowledge that I have read, understood, and agree to the terms and conditions stated herein. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
